The Alpha GPC Stroke-Risk Study, Explained Without Panic
A huge records study raised a flag about a popular choline supplement. Here is how to read it.
What alpha GPC is used for
Alpha GPC (choline alfoscerate) delivers choline to the brain, where it is used to make acetylcholine. In Italy and South Korea it is prescribed for cognitive decline; in the U.S. it is a common ingredient in focus and memory supplements. The strongest trial evidence comes from people with Alzheimer's disease taking 1,200 mg a day[2], supported by a 2023 meta-analysis[3].
The study
Researchers used South Korea's national health insurance database to follow 12,008,977 adults aged 50 or older without previous stroke or Alzheimer's disease. About 109,000 had been prescribed alpha GPC in 2006 to 2008. Over roughly ten years of follow-up, users had a higher rate of stroke than non-users; after matching users and non-users on many characteristics, the adjusted risk was 43% higher for any stroke, with higher risk at longer use[1]. The authors suggested a link to TMAO, a gut-bacteria byproduct of choline that has been associated with vascular disease.
Why it does not prove alpha GPC causes strokes
This was an observational study. People prescribed alpha GPC were older and likely had early cognitive or vascular problems that also raise stroke risk, and no amount of statistical matching can fully remove that. It measured prescriptions, not actual use. And it concerned medical doses, typically 400 to 1,200 mg a day, for months or years. The finding is a reason for caution, not a proven harm.
Practical takeaways
People with a history of stroke, atrial fibrillation, uncontrolled blood pressure or other strong vascular risk factors should talk to a doctor before taking choline supplements at high doses. Everyone else can weigh the signal against the modest evidence for benefit in healthy people. Controlling blood pressure, which reduces both stroke and dementia risk, remains far more valuable than any supplement[4].
What it means for MindVault
Alpha GPC is the sixth of eight ingredients in MindVault's reference blend, which caps it at about 42 mg, a small fraction of the doses in the Korean study and in the Alzheimer's trial. At that amount, neither the benefit nor the risk signal is likely to apply. The bigger safety questions for MindVault are huperzine A and caffeine.
Questions on this topic
Does alpha GPC cause strokes?
That has not been shown. A large observational study linked prescribed alpha GPC with higher stroke risk, which is a signal for caution, not proof.
Is the alpha GPC in MindVault a concern?
At no more than about 42 mg in the reference blend, far below the studied doses, it is unlikely to be the main safety issue.
